Following a short period of absorption of round 30% of the fat, the remaining fat gathers its own blood supply and becomes living tissue.
It is the fat transfer that has the biggest impact on penis size because the penis is thicker in the flaccid and erect state, the girth gains are usually between one and two inches.
When the penis is erect the fat also spreads longitudinally and pushes against the head of the penis. This can also make the penis a little longer.
Following a combination of length and girth surgery the flaccid penis is virtually double the pre-operative size in many cases.

Risks and Complications

Most Patients have an uneventful recovery and after six weeks they are back to resuming normal activity. However all surgery carries some element of risk of course, and it’s important that you understand what these are.
As with any surgery there is always a risk of infection. This risk is extremely low since you are discharged from hospital with a course of antibiotics. However, infection can still happen and this will need to be treated if an infection occurs.
All patients will experience some bruising and swelling after the surgery, this is to be expected and this will settle in the following weeks. Occasionally it can take some time to settle completely and some numbness may remain in areas such as the inner thighs for several weeks or months.
Minor lump and bump formations can happen after the transfer of fat. You will start a period of post op massage of the penis for six weeks to help keep everything smooth and even. Despite this, some lumps can form sometimes. If they become troublesome a visit to the outpatient will clinic will be needed to treat them.
After penis thickening surgery the foreskin usually swells. This is normal and is due to the volume of fat first injected into the penis. Over the following weeks as the fat settles down, you will be able to retract the foreskin over the head of the penis. There is a very rare complication where you cannot , and a circumcision will be required. However this is very rare. Those who are circumcised cannot have this problem of course.
It is worth mentioning here that it is very important to understand from your surgeon what will happen when the suspensory ligament has been cut. This is because if the ligaments are left without even a silicone buffer in place, they will reattach at some point. This can lead to the ligaments attaching higher up. And leaving the penis shorter than the pre surgery length.
If a silicone buffer is used in the surgery ( which is certainly better than nothing ) there is a small chance that the implant may become dislodged from its attachment to the pubic bone. If this happens you may feel a bump at the base of the pubis. Although this may not be painful it would be very annoying and in such cases they are almost always removed. Similarly, if you get a bad infection that is not controlled by antibiotics the implant may also need to be removed. These eventualities mean further surgery of course.
