Penis Enlargement Surgery or Penis Fillers: Which Is Right For You ?

If you’re considering penis enhancement, one of the biggest decisions is whether to choose surgery or penis fillers. Both options have benefits, but the right choice depends on your goals, budget, and recovery preferences. At Moorgate Andrology, we guide men through every step so they can make an informed choice safely and confidently. There is no wrong choice. Both options will increase your penis size, but the decision to go for the penis filler or penis enlargement surgery option, will be decided on a process of elimination

Penis Enlargement Surgery

Surgery is a permanent option that can increase both length and girth. Typical procedures include:

  • Lengthening surgery – releasing the suspensory ligament to allow more of the penis to extend externally. At Moorgate Andrology we do this without using implants. We use a procedure called " Tissue Interposition" which uses a native tissue barrier to stop ligament reattachment. We are the only UK practice doing this because it was developed by our own Urologists. This has been a massive step forward in penile lengthening

  • Girth enhancement – adding volume using a transfer of purified fat taken from your belly and inner thighs. This is the permanent way to increase penis girth compared to the temporary nature of Penis Fillers.

Pros of Surgery:

  • Permanent results. Nothing else lasts longer than Penis enlargement surgery

  • Can address both length and girth. penis Fillers only really address Girth. If you want it longer, this is the route to choose.

  • Performed in a CQC-registered hospital under safe conditions.  And by GMC Specialist Urologists

Cons of Surgery:

  • Higher cost Initially. Length and girth surgery ( most popular choice) costs £ 8495

  • Longer recovery. No sexual activity for six weeks. It takes time to settle down.

  • Surgical risks. All surgery carries risks but these are established at your consultation.

Penis Fillers

Penis fillers
are non-surgical, temporary enhancements using hyaluronic acid to increase girth. Treatments are quick, performed under local anaesthetic, and results are immediate. We brought penis fillers to the UK about 12 years ago. No other clinic has our experience as we have perfected the technique used by most clinics today over the years that we have been perfecting the injection technique and products used

Pros of Fillers:

  • Affordable (starting under £1,000). It costs less than surgery, but only in the short term

  • Minimal downtime. Back to sexual activity in one week compared to six weeks with surgery

  • Reversible if needed. We can inject an enzyme to dissolve it if required

  • Natural-looking girth improvement. A subtle increase in penis size can be achieved with moderate amounts injected

Cons of Fillers:

  • Results are temporary (12–24 months). It costs considerably more than Penis enlargement surgery in the medium to long term.

  • Cannot significantly increase length. It might look a touch longer because of the weight of the filler on the penis, but its really all about the penis girth

  • Will require top-up treatments to maintain results. Hyaluronic acid absorbs over time. On going treatments are required to maintain the results. If you stop the tops ups the penis size will reduce again.

Choosing the Right Option

When deciding between surgery and fillers, consider:

  • Your goals: Want permanent change in both length and girth? Surgery may be best. Want a temporary, low-risk girth boost? Fillers are ideal. Do you want the maximum size increase ?, surgery might be best. More subtle increase ?, fillers can achieve this

  • Budget: Surgery is more expensive, while fillers are more accessible financially. But consider longer term before deciding.

  • Recovery time: Surgery requires six weeks to fully recover; fillers only require one week with no sexual activity.

  • Risk tolerance: Both are safe in professional hands, but surgery carries slightly higher risk due to its invasive nature. That is not to say that Penis fillers dont have risks because they do.

At Moorgate Andrology, we provide a consultation to assess your anatomy, goals, and lifestyle, helping you choose the option that delivers the best results safely.

Combining Surgery and Fillers

Some men choose fillers first, then later move to surgery for more permanent results. This allows you to:

  • Test how enhancement feels and looks.

  • Adjust your goals before committing to surgery

  • Maintain confidence while planning a longer-term solution

About 40% of all patients who start with penis filler eventually go on to have penis enlargement surgery. This is for a variety of reasons but usually because they get fed up of paying for top up treatments. Other reasons include more confidence in the whole process and timing in terms of having the funds to do the surgery
