For men considering penis enlargement, one of the biggest questions is: “What exactly happens during the surgery?”
At Moorgate Andrology, we believe in transparency. Understanding the step-by-step procedure helps men make confident, informed decisions. This guide explains what happens before, during, and after penis enlargement surgery, so you know exactly what to expect. This explanation is pretty much what I discuss with all patients who call me to ask about the procedure.
Consultation: Your First Step
Every journey begins with a confidential consultation with one of our experienced urologists. At this appointment, you’ll:
-
Discuss your goals – whether you want length, girth, or both. ( 80% of patients choose length and girth together)
-
Review your medical history – ensuring surgery is safe for you. ( Your safety is our number one concern)
-
Examine your anatomy – to set realistic expectations. ( Some patients want additional procedures at the same time such as pubic liposuction of fixing of buried penis )
-
See before-and-after results – from our gallery of real patients. ( spend some time checking out whats achievable)
-
Ask questions – about risks, recovery, and outcomes. ( Get all the facts, and we mean all the facts, we tell you as it is )
This consultation ensures that you understand what surgery can achieve for you personally. In fact, we will arrange two consultations with our Urologists before your surgery takes place. Just so you have plenty of opportunity to ask anything and understand everything
Pre-Operative Preparation
If you decide to go ahead, we’ll book your surgery date and give you a pre-op checklist. This usually includes:
-
Standard swab test and general health checks. If any additional tests are needed we will tell you
-
Stopping smoking, as it slows healing. It certainly helps if you can, or at least try to cut down a bit
-
Avoiding certain medications (e.g. aspirin, blood thinners). We will review your medication with you
-
Instructions for fasting before your operation. We provide clear pre operative instructions that are easy to follow
Our team will guide you through every step so you arrive prepared and confident. We even have our own dedicated pre and post op team who you will have access to anytime before and after your surgery
The Day of Surgery
On the day of your procedure, you’ll be admitted to one of our CQC-registered hospitals. Here’s what happens:
-
Admission and preparation – you’ll meet your surgeon and anaesthetist, and final checks will be done. Generally, you are admitted about two hours before the procedure,
-
Anaesthesia – surgery is usually performed under general anaesthesia (you’ll be asleep).
-
Lengthening surgery – the suspensory ligament is released to allow more of the penis to extend outside the body. We do this without any implants and we are the only UK clinic that can.
-
Girth enhancement – surgical techniques are be used harvest fat from the belly and inner thighs, purified and then injected into the shaftt o increase circumference.
-
Closing and dressing – incisions are closed carefully for minimal scarring, and a sterile dressing is applied.
The procedure typically takes 1 hour, depending on whether you choose lengthening, girth, or both.
Immediate Recovery
After surgery, you’ll be taken to a recovery suite where:
-
Nurses will monitor your vital signs.
-
You may feel mild discomfort, managed with pain relief.
-
Swelling and bruising are expected but temporary.
All patients go home the same day. Expect t be at the clinic for about 5 to 6 hours in total.
Recovery Timeline
-
First 1–2 weeks – swelling, bruising, and mild discomfort; rest and follow aftercare advice. Take a few days off , or maybe a week if your job involves heavy lifting
-
Weeks 3–6 – swelling reduces, shape begins to normalise; light activity resumes.
-
After 6 weeks – most men return to sexual activity.
-
3 months+ – final results visible, permanent changes in size.
We provide a full aftercare plan, including follow-up visits, to ensure your recovery is smooth.
What Results Can You Expect?
Results vary from man to man, but on average:
-
Length: 1–2 inches increase in the flaccid length
-
Girth: 1–2 inches increase in both the flaccid and erect girth
Unlike fillers, which only provide a temporary boost, surgery offers permanent enlargement in both length and girth.
Risks of Surgery
As with any medical procedure, there are risks:
-
Infection (rare, prevented with antibiotics). About 1% of our patients get an infection that needs further treatment
-
Swelling and bruising (expected).
-
Temporary reduced sensitivity on the belly and inner thighs where the fat was taken from.
-
Scarring (minimal with expert surgeons). The lengthening scar is now much smaller and hidden neatly in the natural fold at the base of the penis where your penis meets your body
At Moorgate, all surgeries are performed by specialists in CQC-approved theatres to keep risks as low as possible.
Why Choose Surgery Over Fillers?
Fillers are often promoted as “quick fixes,” but they:
-
Only increase girth (not length).
-
Last 12–24 months, requiring top-ups.
-
Can cause lumps or unevenness if not performed by experts.
Surgery, on the other hand, offers permanent, natural-looking results in both length and girth. That’s why Moorgate Andrology promotes a surgery-first approach.
